Only customers residing in the state of Michigan are eligible ("Eligible Customer"). Vehicle sale or lease must be registered in the name of the Eligible Customer.
May combine with National Customer Rebate, Loyalty, Military, Special APR, or Special Lease programs if applicable. All Customer Rebates will be paid to the selling Dealership, unless otherwise noted by the Buyer, on each Eligible Vehicle retailed from unit one during the Program Period as set forth below. Customer rebates will not be paid to the selling Dealership if Eligible Customer indicates that he or she is to receive a direct payment for the rebate amount on the Rebate Program Form. Sales count toward volume objective, and pay under MiDealer Rewards (MDR), and Diamond Rewards programs. Eligible Customers must sign contract and take delivery within the Program Period. Eligible Customers have the option of applying the Rebate to be paid directly to them via rebate check from MMNA or apply the amount of the Rebate towards the purchase or lease of an Eligible Vehicle (payment assigned to Dealership). PROGRAM SPECIFIC FORMS AND DOCUMENTATION: The Program Specific Forms and Documents required must be completely filled out, signed by both the Eligible Customer and the Dealership, and uploaded to the Incentives Claims System (ICS) on MDL within ten (10) working days of retail purchase or lease to be eligible for payment. Failure to submit these completed forms will result in delays in processing. A completed copy of the "Mitsubishi Motors Universal Rebate Form (v. 04/26)" signed by the Eligible Customer and the Dealer to show the rebate offer was presented to the customer and how the customer chose to apply the rebate(s). A photocopy of the Michigan driver license showing that the Eligible Customer is a resident of the State of Michigan. In addition to a photocopy of the Eligible Customer's driver's license, dealership will also need to submit additional proof of residence documentation which could include a copy of a current utility bill (including but not limited to electric bill, water bill, gas bill, or landline phone bill) a fully-executed current lease or rental agreement, a pay statement with customer's name and address, a Michigan voter registration card, or any other government-issued document conclusively showing that the Eligible Customer is a Michigan resident. A completed copy of the Retail Buyer's Order and/or Financing Contract signed by the Eligible Customer and the Dealer disclosing the applicable State of Michigan Resident Customer Rebate amount as a separate line item and how it was applied to the sale transaction (if applicable). IMPORTANT: All forms and documents specified under the Program Specific Forms and Documentation section of this program must now be uploaded to Incentives Claims System (ICS) on MDL within 10 working days of retail purchase or lease to be eligible for payment. All claims must be fully approved with proper documentation within 90 days of sale date to be eligible for incentives. Program ID MICR0926 must be used.
